(Please complete all applicable items in Sections II through VII. Descriptions of your school organization and education plans should be as detailed as possible. Avoid using generic curricula and descriptions of other schools or programs. Be specific in describing your plans. Applications should be typed double-spaced and organized by sections with items numbered accordingly.)
1. Specify the number of students to be served, by grade level. If the proposed charter school would serve fewer than 50 students at a single site after the first year of operation, a compelling justification is required. Please provide such justification.
2. Describe the ages and grade levels of the students to be served.
3. Describe all of the proposed admission policies and procedures for the charter school, which must be consistent with the provisions of Section 2854(2) of the Education Law. Provide copies of all such policies and procedures.
4. Describe all of the requirements students will have to meet in order to receive a diploma (if applicable). Include all course and testing requirements.
5. Describe the methods and strategies by which the school will assure the provision of services to students with disabilities in compliance with all federal laws and regulations, including a description of:
a. the enrollment method that will ensure equal access for students with disabilities;
b. the special education services to be provided directly by the proposed charter school and those to be provided by the student’s district of residence (e.g. consultant teacher services, related services, resource room, special class);
c. how the proposed charter school will interact with the student’s district of residence to ensure that the special education services listed on the student’s IEP are provided. Who will have this responsibility and how often will they interact with the school district?;
d. how the proposed charter school will ensure that the teacher(s) of a student with a disability will participate in meetings of the Committee on Special Education, and will have access to and understand their responsibility to implement the IEP;
e. the processes to be followed by the proposed charter school that will comply with the Child Find requirements of IDEA (34 CFR 300.125);
f. the processes to be followed by the proposed charter school to comply with the data reporting requirements of IDEA;
g. the processes to be followed that will ensure compliance with the Family Educational Rights and Privacy Act (FERPA) and IDEA regarding confidential student records (e.g. notice to parents of rights, record access log, training of personnel, maintenance of student records, records official); and,
h. the processes that will be followed by the proposed charter school that will ensure that parents are kept informed of their child’s progress in meeting his/her IEP goals (34 CFR 300.347 – at least as often as parents are informed of their non-disabled children’s progress).
6. Describe the methods and strategies by which the school will serve students with limited English proficiency, by grade level, including:
a. the process for identifying LEP/ELL students and the method for determining an appropriate instructional program for these students;
b. the special language instructional program the school will use to teach LEP/ELL students English and to teach the general curriculum;
c. a provision to indicate that LEP/ELL students will not be excluded from curricular and extracurricular activities in school because of the inability to speak and understand English;
d. a plan to ensure that needed staff, curricular materials, and facilities to serve LEP/ELL students are in place and use properly;
e. appropriate evaluative standards for measuring the progress of LEP/ELL students, including program exit criteria;
f. a process for continued program assessment and modification where needed;
g. methods and strategies to ensure that LEP/ELL students are not assigned to special education classes because of their lack of English proficiency; and
h. an assurance that all parents, including those whose English is limited, will receive notices and other information from the school in a language they can understand.
7. Describe plans for maintaining student discipline, including rules and due process procedures for disciplining, suspending, or expelling students, and the process for providing students of compulsory attendance age with alternative instruction if they are suspended or expelled (until enrolled in another school). Include methods and strategies for assuring that students with disabilities are provided with appropriate due process and a free appropriate public education per the requirements of IDEA and 34 CFR §§ 300, et seq. Provide copies of all such policies and procedures.
8. a. Describe the health services to be provided to students. If such services will be contractual, provide a copy of the draft contract or letter of agreement. Be sure to include such expenditures for health services in the budget.
b. Describe the food services to be provided to students.  Sweetened foods cannot be sold prior to the last scheduled meal period, per Education Law 915. Provide copies of any contracts.
