Teacher Certification and Employee Records for Charter Schools

Pursuant to New York State Education Law, the No Child Left Behind Act of 2001(NCLB) and the Individuals with Disabilities Education Act (IDEA), New York State Education Department (SED) is requesting that all charter schools provide reports and supporting documentation concerning teacher certification and employee records.   Please download the form and instructions by clicking the links below. Please read the instructions carefully and complete all applicable tables. All tables must reflect data as of October 3, 2007. All tables and supporting documentation must be submitted no later than October 17, 2007. 

The form consists of five Microsoft Excel tables. Each table can be accessed by clicking the color-coded tabs at the bottom of the Excel screen.  All tables must be completed by all charter schools, with the exception of Table B which may not apply to some schools.  Following is a summary of the five tables.

TABLE A – Professional instructional staff
TABLE B – Questionnaire for uncertified teachers with three years of classroom experience
TABLE C – Non- instructional staff
TABLE D – Newly hired and terminated employees
TABLE E – Highly qualified teachers

Self Assessment System for Schools

In accordance with the No Child Left Behind (NCLB) legislation, Adequate Yearly Progress (AYP) must be determined for each public school and charter school in the State.  To meet this requirement of NCLB, Commissioner’s Regulation Part 100.2(p) requires that a self-assessment be conducted in those schools for which decisions regarding AYP cannot be made based upon students’ participation in State assessments.

For the school/district that failed to make AYP in 2005-06 or was in need of improvement status for 2006-07, the deadline to e-mail completed form is August 31, 2007.  For the school/district that made AYP in 2005-06 and/or was in good standing status for 2006-07, the deadline to e-mail completed form is September 14, 2007. Forms should be e-mailed to selfassessment@mail.nysed.gov.
