Statewide School Accountability

 

There are three sets of forms available at this site.

1. If you are a Charter School located in New York City and have received a request for Teacher Certification and Employee Records, please proceed to section 1

2. If you have received notification for completing a Self Assessment for 07-08, please proceed to Section 2

3. If you have received notification for completing Alternative Accountability system, please proceed to Section 3

Section 1: Teacher Certification and Employee Records for Charter Schools located in New York City

Pursuant to New York State Education Law, the No Child Left Behind Act of 2001(NCLB) and the Individuals with Disabilities Education Act (IDEA), New York State Education Department (NYSED) is requesting that all charter schools provide reports and supporting documentation concerning teacher certification and employee records.   NYSED is also requesting all charter schools to assure that the submitted data is complete and accurate. Please download the Teacher Certification and Employee Records Form, Assurances Form and Instructions by clicking the links below. 

PLEASE READ THE INSTRUCTIONS CAREFULLY AND COMPLETE ALL APPLICABLE TABLES
.  All tables must reflect current data.

The Teacher Certification and Employee Records Form consists of seven Microsoft Excel tables. Each table can be accessed by clicking the color-coded tabs at the bottom of the Excel screen.  All charter schools must complete Tables A, F and G.  Charter schools with uncertified teachers who posses a qualifying exemption must complete Tables B, C, D or E.  Following is a summary of the seven tables.
TABLE A – Professional instructional staff
TABLE B – Questionnaire for uncertified teachers with three years of classroom experience
TABLE C – Questionnaire for uncertified teachers who are tenured or tenure track college faculty
TABLE D – Questionnaire for uncertified teachers with two years of TFA experience
TABLE E – Questionnaire for uncertified teachers with exceptional experience
TABLE F – Non- instructional staff
TABLE G – Newly hired and terminated employees

DO NOT ATTEMPT TO COMPLETE THE TABLES WITHOUT REFERRING TO THE INSTRUCTIONS.
 
The Teacher Certification and Employee Records Form and Assurances Form, along with supporting documentation, must be submitted to NYSED no later than January 16, 2009.  Both forms must be emailed to emscapa@mail.nysed.gov.  Additionally, all supporting documentation along with the paper copies of the two completed forms must be mailed via postal service to the address provided in the online instructions.

Section 2: Self Assessment System for Schools

In accordance with the No Child Left Behind (NCLB) legislation, Adequate Yearly Progress (AYP) must be determined for each public school and charter school in the State.  To meet this requirement of NCLB, Commissioner’s Regulation Part 100.2(p) requires that a self-assessment be conducted in those schools for which decisions regarding AYP cannot be made based upon students’ participation in State assessments.

The deadline to e-mail the completed form for elementary and middle schools is June 11, 2009 and for high schools it will be August 6, 2009. Forms should be e-mailed to selfassessment@mail.nysed.gov.
