4.	Title of the instructional program, curriculum series to be used (if appropriate):	We use Curriculum Associates’ Strategic Steps to Mathematics Success Series and Three Steps to Reading Success in conjunction with supplementary materials that comply with the New York State Learning Standards.

7.	Number of sessions provided per week, if applicable:	There are several options: One, two-hour session per week in small group environment Two, two-hour sessions per week in a small group environment One, one-hour session per week in a one-on-one environment

10a.	Description of methods and strategies for serving eligible students with disabilities:	We have certified Special Education teachers on staff who utilize their skills and knowledge to serve students with disabilities.

Qualifications of the service provider(s)/instructor(s):	Certified teachers will provide instruction and use evidence-based instruction techniques to plan and deliver effective instruction for students who want to enhance, or need to improve, their English Language Arts (including reading) and math skills. Teachers will regularly attend training seminars in order to enhance their skills and to keep their information current.
12.	Reports to parents, teachers, and LEA (Content, frequency, method of delivery):	Reports will be sent throughout the duration of the program.  The first and last reports will be based upon diagnostic and post test results.
