3. Evidence that services have been effective: Increased NYC Standardized Test Scores of our students and Very Positive Parent/Child satisfaction surveys of our parents and students.
4. Title of the instructional program, curriculum series to be used (if appropriate): In our Family Centered Tutorial Project for Reading we will be using the Voyager Universal Literacy System. Not appropriate for our after school program or for Math.
5. Grade level(s) the provider will serve: K-8 in the Family Centered Tutorial Project and at PS 174 After school program. K-5 for PS 306 After school program.

7. Number of sessions provided per week, if applicable: 2 - One hour tutoring sessions and one weekly Parent Group session for students participating in the Family Centered Tutorial Project. For the Gary Klinsky After School Programs at PS 306 and PS 174 3PM-6PM 5 days per week when school is in session.
8. Average length of each service session (minutes/hours): One hour indivdual tutoring twice per week and 1.5 hours parent group in the Family Centered Tutorial Project. Three hours per day when school is in session in the Gary Klinsky After School Programs at PS 306 and PS 174

Duration of the available service period (e.g. September 1 – June 30; September 1 – August 31; weekends only; summer school only; specific number of weeks or hours, other, etc;): Mid September through Mid June - Monday through Thursday in the Family Centered Tutorial Project. September 10 - June 24 when school is in session in the Gary Klinsky Children's Center at PS 306 and 174
10. Type of instruction: One on One/Two tutoring in the Family Centered Tutorial Project; Small group instruction using a multi-sensory, cooperative learning approach in the GKCC after school programs at PS 306 and PS 174.
11. Qualifications of the service provider(s)/instructor(s): BBCS has been providing services to Brooklyn residents since 1866. We have extensive experience operating early childhood, after school and family counseling programs as well as programs for men and women with emotional, cognitive or physical disablities. Volunteer tutors are college students or professional adults and are supervised by a NYS Certified Teacher in the Family Centered Tutorial Project; NYS Certified Teachers and Assistant Teachers who work together as a team in the GKCC after school programs at PS 306 and PS 174.
12. Reports to parents, teachers, and LEA (Content, frequency, method of delivery): Parents and Teachers: Every three months progress report in face to face meetings. LEA : Monthly attendance report and every three months written report which would identify progress toward goals achieved including results of pre/post testing.
13. Will transportation to the service location be provided at no charge if requested? (Note: LEAs are not required to provide or pay for transportation) No.
14. Additional relevant information: Our services are highly effective. Our goal in working with the family is to assist our families in developing the tools to achieve academic success. We believe that parent involvement is a key tool in this process.
