Teacher Certification and Employee Records (2008-2009)

 

The Teacher Certification and Employee Records Form must be accurate and complete because it will be used to satisfy State and Federal reporting requirements.

 

In order to process the Teacher Certification and Employee Records Form from charter schools, New York State Education Department must receive a copy of the attached Assurance Form signed by the school’s chief administrator.
