Verify Candidate Status by Mail

Verifications of Diploma Issued

If you need to verify that a New York State High School Equivalency diploma was issued to a candidate, you must complete Attachment G and mail it to the address at the top of the form.

At this time, the NYS GED Testing Office only accepts and processes requests for diploma/transcript verification via our standard form, Attachment G. Once all information on the Attachment G has been completed, the form must be mailed to our office at the address located on the top of the form.  Faxed requests will not be accepted or processed.

Please note that we do not accept verification requests on any other form other than the Attachment G, although agencies requesting verification may submit their own standard Release of Information Waiver signed by the applicant in addition to the completed Attachment G.

Due to the enormous amount of verification requests received by the GED Testing Office, please allow for 4-6 weeks processing time for each verification request.
