Instructions for Completing
the Application for GED Testing (Attachment A)
Item 1. Clearly and NEATLY fill in
the candidate's complete Social Security number. If the candidate does not have a Social Security number, leave
this blank and an identification number will be assigned. This identification
number will appear on candidate test results.
It is important that the candidate keep this number for future reference
since it will become the permanent record identification number.
Item 2. If the candidate is in a GED test preparation program, ask the teacher for the program name and code number to put in this item. If the candidate is not in a program, leave this item blank.
Item 3. Print candidate name in this order: last name, first name, and
middle initial.
Item 4 and 5. Print candidate mailing address. Make sure all information is complete (P.O. Box, Apartment Number, Zip Code, etc.). If the candidate moves after sending this application, candidate MUST contact the local testing center to change candidate record (see Section 12 for list of testing centers.)
Item 6. Print candidate telephone number beginning with area code.
Item 7. Print candidate date of birth using the number of the month, the day and the four numbers of the year. For example, a person born on October 8, 1969, would put in his or her date of birth as: "10/08/1969 (10th month, 8th day, 1969).
Item 8. Put candidate current age (how old candidate is on the day he or she fills out the application).
Item 9. Use a check mark (4) to indicate if candidate is a male or a female.
Item 10. Use a check mark (4) to tell us in which language the candidate wishes to be tested. The tests are given in English, French and Spanish.
Item 11. Print the name and address of the last school K-12 candidate attended. This does not include candidate's GED Test preparation program.
Item 12. Use a check mark (4)
to answer "Yes" if
candidate previously tested in New York State or "No" if candidate
did not. If candidate checks
"Yes," complete items 13-17.
If candidate checks "No," go to item 18.
Previous exam information
If the candidate has taken the GED test before in New York State, provide all information requested in items 13-17. The candidate can obtain this information from the Ineligible Notice/Unsatisfactory Score Report he or she received when candidate last took the GED test.
Item 13. Print the name candidate used when taking the exam, beginning with last name, followed by first name and middle initial.
Item 14. Print the identification number the candidate used at the test, e.g., SS number or GED assigned number.
Item 15. Print the name of the center where the candidate took the test. If candidates do not know where they tested, give the approximate location.
Item 16. Print the date candidate tested. If the candidate does not know when he or she tested, give the approximate time period.
Item 17. Print the form(s) of the test(s) taken (e.g., AT, AR, AQ, etc.) This information is also on candidate score report form, if candidate is a retester.

to indicate if candidate is requesting special modifications to take the test.

Item 21. If candidate is 19 years old or older, check "Yes" and
go to Item 23. If the candidate is 18
years of age or older, check "No" and go to Item 22.

Item 22. Darken ONLY ONE eligibility
category and attach documentation to prove the candidate meets the
criteria.
Permission to release GED
test scores
Item 23. Read the permission statement.
If the candidate wants to have his or her test results/scores given to
candidate GED preparation program or test center, check (4) "Yes." If candidate does not, check (4)
"No." By checking
"Yes," the candidate will be helping candidate GED preparation
program to improve its services. Sign
candidate name and put the date the candidate is completing the application.
Certification/Affidavit
Item 24. Carefully read each item of the affidavit information. By signing this affidavit, the candidate is stating that he or she has given correct, truthful and complete information on the application and any attachments candidate may have provided. Review candidate application and attachments to be sure ALL information is correct and sign candidate name.
Permission of parent/guardian
Item 25. If the candidate is under 18, he or she must have his or her parent/guardian fill out the required information and sign and date the application.
If candidate is under 19 years
old, attach all required proof of test eligibility (see Section 2).
