Petition for Registration of a Public School

I, the undersigned, hereby petition for the New York State Board of Regents pursuant to Part 100.2(p) of Commissioner’s Regulations to register the school identified below as a school effective on the following date _________________.

I certify that as of the effective date of the registration of this school:

1. The Board of Education of this school district has adopted a resolution seeking permission from the Board of Regents to establish a public school (see attached).
2. The school will comply with applicable Federal and State law and regulations pertaining to public schools.
3. The school facility will operate in accordance with all applicable building codes for public schools in this school district and the district will have secured an appropriate Certificate of Occupancy to operate the school on the effective date of the registration.  I agree to submit a copy of such Certificate of Occupancy no later than 30 days after the commencement of school operations.
4. The school will be assigned a full-time principal who is under the direction of a superintendent and holds the appropriate certification as required pursuant to section 80.4(b) of Commissioner’s Regulations.
5. The school will provide all students with instruction in the learning standards, employ certified teachers, administer mandated State tests or approved alternatives, and provide at least 180 days of instruction as is required by Commissioner’s Regulations.
6. Students registered at the school will not be on the registers of any other school, public or private.
7. The district agrees to report all applicable data for the school using the school’s unique BEDS code and disseminates a unique report card developed or approved by NYSED.
8. The district acknowledges that the school is subject to the school accountability provisions of Commissioner’s Regulations and that the registration of the school may be revoked by the Board of Regents pursuant to the process detailed in Part 100.2(p) of Commissioner’s Regulations.
