  • Type or print your return address legibly, and make sure to include your zip code.
  • Requests must be accompanied by check made payable to the New York State Education Department.  The cost of the publication is listed with each publication entry.  If there is no price listed, that particular publication can be obtained without charge.
  • Postage and handling charge is $3.00 for orders of up to 5 publications.  Add $1.00 per each additional 5 publications ordered.  If your order is over 50 publications, please call (518) 474-3806 TO VERIFY POSTAGE AND HANDLING CHARGES.
